Make a representation for not filing an identity verification statement

Making a representation means telling us about exceptional circumstances you want us to consider before we take enforcement action for an overdue filing. You must do this within 28 days of the date on the default letter you received.

You can only use this form to make a representation for not filing:

  • a confirmation statement and director identity verification statements
  • an identity verification statement for a person with significant control (PSC)

If your representation is accepted, we'll pause enforcement action to give you time to file.

To complete this form, you'll need to provide:

  • your full name and email address
  • the company name
  • the company number
  • the date on the default letter you received
  • information about the exceptional circumstance

You can also upload up to 2 pieces of evidence to support your request.
