How to complete this form

Part 1: Security questions

Your answers must be the same as what you told us on your online claim. You will need the following information to hand:

• claim reference number
• National Insurance number

Part 2: Amendment section

You will need the following information to hand:

• gross weekly pay
• employment start and end date
• notice period start and end date

We do not need any other information about your employment details. Even if you told us something else incorrectly in your claim, it will not affect how we calculate what you’re owed.
