NHS number

We will need your NHS number and date of birth to be able to remove you from the study. You can find your NHS number on any letters received from the hospital or your GP or by logging into the NHS app.

Please ensure your NHS number has been entered correctly as we will need this to be able to locate your records to remove you from the study.
