Penalty enforcement form for an Acas COT3

Apply for a penalty for an unpaid Acas settlement

Before you start

You'll need:

  • a copy of the Acas agreement (COT3)

  • the amount of the settlement, including any separate costs 

  • the date payment was due 

  • how much the respondent paid, if they paid part of the settlement 

  • the respondent's contact details

Make sure it's the final version. This will have the Acas logo at the top. Do not upload draft versions

  • Microsoft Excel Spreadsheet (.xlsx)
  • Microsoft Word Document (.doc, .docx)
  • PDF (.pdf)
  • text (.json, .odt, .rtf, .txt)

Your file must be smaller than 7MB.
