You can use this form to share feedback and concerns about how public authorities deliver their Prevent duties (opens in new tab).

This will help us identify and review issues with how the duty is applied.

The Office of the Independent Prevent Commissioner will not investigate every concern individually, but will record all of them and look for patterns. These records will contribute to future reviews.

The form should take 15 to 20 minutes to complete.
