Request to extend the identity verification deadline for a person with significant control (PSC)

Use this form if you're a PSC, or filing on behalf of one, and you want to apply to extend your deadline for meeting identity verification requirements for Companies House.

If you're a PSC of more than one company, you'll need to submit separate requests for each PSC role.

When to submit your request

You must submit the request before your current verification deadline has passed.

Only use this form if you've already been given two extensions using the 'Provide identity verification details for a person with significant control' service (opens in new tab).

What you'll need

To complete this form, you'll need to provide:

  • your full name, as it appears on the Companies House register

  • the company number for the company you are a PSC of

  • a reason for the extension request

  • supporting evidence

Supporting evidence

You must upload supporting evidence, or your request will be rejected. You can upload a maximum of 2 documents.

The evidence you need to provide depends on the reason for your extension request. The following guidance provides examples of evidence you can upload.

If you've applied for the identity documents needed for verification, but they’ve not arrived yet

A document or screengrab that shows you've applied for identity documents. It must show a time and date. For example, an email confirmation of a passport application must show the time and date the email was received.

If you've arranged to verify identity at the Post Office

Evidence that you've used the GOV.UK One Login service to arrange to verify your identity at the Post Office. For example, the confirmation email or Post Office customer letter that you were sent by GOV.UK One Login. It must show the date it was sent to you.

If you're undergoing medical treatment that means your identity cannot be verified on time

A document from a recognised medical professional, such as a GP or hospital, that confirms your medical treatment. It must include an official letterhead or verifiable stamp.

If you're unable to verify your identity without assistance, and have arranged additional support

A document confirming the reason you need additional support. This can be a copy of an email, letter or report. It must be from either an educational professional, education psychologist, medical professional, occupational therapist or online screening product.

If you're experiencing technical issues for a prolonged period of time, which means your identity cannot be verified on time

An email or screengrab from a service provider outlining the technical fault and the date and time that it occurred.

If your PSC details on the company record are wrong, and more time is needed to file a correction

A document or screengrab that shows a corrections filing has been submitted. This can include a copy of the form, or something that shows the reference number for the filing.

After you've submitted the request

We'll review the evidence and email you to let you know if your request has been accepted or rejected. This decision is at the registrar's discretion.

If your request is accepted, you'll have an extra 14 days to provide your identity verification details, starting from the end of your current 14 day period.
